The purpose of the nutrition assessment is to evaluate your current eating habits, discuss areas of concern, and provide education regarding healthy nutrition & eating patterns. The assessment will include: completion of the nutrition questionnaire prior to the appointment, questions about your typical days food intake, review of dietary guidelines and recommendations, and individualized education about any areas of concern and the importance of eating healthy and consistently.
